INCOMING STUDENT-ATHLETE MEDICAL PACKET

ALL INCOMING STUDENT-ATHLETES MUST COMPLETE THE MEDICAL PACKET LISTED ABOVE AND ATTACH THE FOLLOWING DOCUMENTS:
1.       COPY OF INSURANCE CARD - FRONT AND BACK
2.       COPY OF SICKLE CELL RESULTS
 
YOU MAY FAX THE COMPLETED PACKET TO THE ATHLETIC TRAINING ROOM AT 201-692-2171 (ALL THIS INFO IS ON THE PACKET AS WELL)

Some athletes may have to do the ADD/ADHD or get medical notes/clearance if applicable. If this is the case, the Athletic Training staff will provide instructions and directions to do so.”

Immunization Form - This is collected as a part of your general admission material and should be on file in the FDU Health Center. We do not need copies of these forms. Meningitis immunization is recommended for all students and is required for any student living on campus - even if just for a short time. University Health Insurance - All International Student-athletes must purchase University Health Insurance as well as any Domestic student-athlete who does not have primary/family insurance.
